Vaccinations and Wellness: Have a new puppy or an adult pet that needs a wellness exam? We are happy to provide full-service wellness care. We can do wellness appointments and drop-offs.
Surgery: All of our doctors perform surgeries on a rotating weekly schedule. We perform spays, neuters, dental procedures, limited orthopedic surgeries, and many other procedures. You may fill out your surgical forms ahead of time by visiting the link that will be sent to you as your pet's appointment approaches. If you'd like to fill out the forms on paper, the forms below are available to print:
Boarding: We are happy to provide your pet a home-away-from-home when you are out of town. We can board the largest dogs to the smallest kitties. Check out our FAQ section for what you need for boarding. Our boarding form is available below:
Emergencies: Is your pet having an emergency during our business hours? You're welcome to walk in with your pet in the event of an emergency. Emergencies include, but aren't limited to: pet was hit by a car, trouble breathing, animal fight/attack, etc. It's best to let us know you're on your way. The following form will be requested upon check-in:
New Clients: Are you a new client to SCAH? If so, please print and fill out the following form:
Drop-Off Samples: If your pet is having stool or urinary issues at home, you're welcome to drop off a fecal or urine sample. Keep in mind that your pet needs to be up to date on their yearly exam to qualify. If not, please schedule an appointment. Please see our FAQ section for more information.
